SWISS DOLORCLAST® METHOD
RADIAL SHOCK WAVE THERAPY
FOR WOUND MANAGEMENT
The EMS Swiss DolorClast® generates radial extracorporeal shock waves that are directly applied to acute and chronic wounds under sterile conditions utilising coupling gel. Radial extracorporeal shock waves are characterised by a high positive peak pressure of more than 10 MPa, a fast initial rise in pressure during a period of approximately 1 ms, a low tensile amplitude of approximately-5 MPa, and a short life cycle of approximately 20µs.
Radial extra corporeal shock wave therapy (RSWT) for acute and chronic wounds has a success rate of approximately 75%, can be performed easily in both inpatient and outpatient settings, does not need more than 5 to 10 minutes per treatment session and should be considered an additive therapy option in modern wound management. |

CONTRA INDICATIONS
> |
Contraindications: infected wounds, necrotic wounds and any Grade III wound according to the
University of Texas Wound Classi fication System |
> |
Precautions: be careful when treating ischaemic wounds |
TREATMENT PROTOCOL
> |
Perform standard wound cleaning and debridement prior to RSWT |
> |
Place sterile, plastic drape over the wound and surrounding tissue (the plastic drape should overlap the wound margins by at least 5cm) |
> |
Apply sterile coupling gel onto the drape (do not apply coupling gel between the wound and the plastic drape) |
> |
Apply radial extracorporeal shock waves (two treatments per week; a total of six to eight treatments;
1,000 impulses per Cm2) and treatment session; energy flux density (EFD)=0.08mJ/mm2 ; no preferred treatment of the wound margins) |
> |
Remove gel and plastic drape, clean with sterile saline |
> |
Apply standard wound dressing according to the individual phase of wound healing |
MODE OF ACTION
Extracorporeal shock waves: |
> |
Increase the release of wound growth factor |
> |
Stimulate the expression of angiogenesis - related growth factor |
> |
Promote vessel in growth |
> |
Increase cell proliferation. |
